For customers entitled to claim exemption from VAT because their disability or condition qualifies them to do so, the links below provide a claim form in two formats; PDF and Microsoft Word. You can download your choice of form by clicking the link title.

Please note: As this site is used by people who do not qualify for VAT exemption, all on-line ordering includes VAT. To claim back the VAT, you need to complete a form and return it to us by post, fax or email. When we receive the form a refund payment will be made equal to the amount of VAT applied to your order.
